Live a Healthier You Series – A Diabetes Prevention Program is a year-long program focused on long-term changes and lasting results. It is an evidence based program of the Center for Disease Control. Sessions are tailored and adapted to match attendees background, interests, and needs. Positive lifestyle changes such as healthy eating, increased physical activity, and losing weight lowers the risk of pre-diabetes and type 2 diabetes and improves your overall well-being and the well-being of your family.
This program will have weekly classed for the first 16 weeks; then, once a month for six months.
Participants: Are diagnosed with pre-diabetes or at risk for diabetes
