Keswick is a dedicated partner in our patients’ journey to wellness—even after they leave our campus. The Home & Healthy® program is designed to provide our patients with the services and support they need once they have completed their rehabilitation program and are ready to return home.
Home & Healthy® Support
Available Monday – Friday
8:30 a.m. – 5:00 p.m.
Messages left after hours or on weekends will be returned the next business day.
Once a Home & Healthy member is home, Keswick’s team of professionals will remain in ongoing contact with him or her in order to:
- Answer questions
- Provide information
- Offer encouragement
Members may also call our Home & Healthy® team whenever they have a question or require assistance.
A Connection to Services in the Community
Home & Healthy® can also assist members in finding the services they may desire when they return home. Keswick has a network of trusted community partners who can provide our members with a wide variety of services including:

- Private Duty Care
- Companion Care

- Legal Services
- Home Modifications
By connecting members to these services, Home & Healthy can help them stay in the comfort of their own home—and ultimately avoid returning to the hospital.
Keswick is able to help its guests in accessing additional services they may desire upon returning home. Although a Keswick team member may offer information or assist with contacting these additional services, Keswick assumes no responsibility for the actions or inactions of any service agency or care provider. While Keswick may offer the names of various service providers, the actual selection of these services rests entirely with the guest, or his or her responsible party.
